Yes if its in Baker, Bradford, Clay, Duval, Flagler, Marion, Nassau, Putnam, St Johns, or E. Volusia counties
No, its not required in any of the others, but it is MUCH less paperwork and its much easier! You can instead download the forms, print them, fill them out and then return them to us. PLEASE BE ADVISED THAT THERE IS A $55 FEE FOR US TO PROCESS THE PAPER VERSION OF THE FORMS. If you can't download them we can always fax or mail them to you.
